Sandhills Medical Foundation, Inc. has been providing quality comprehensive health care since 1977. It is a Federally qualified community health center (FQHC) serving residents of Chesterfield, Kershaw, Lancaster, and Sumter Counties. Community Health Centers are community-based and patient-directed organizations that serve populations with limited access to health care. These include low income populations, the uninsured, those with limited English proficiency, migrant and seasonal farmworkers, individuals and families experiencing homelessness, and those living in public housing.

As a patient centered medical home provider, Sandhills Medical Foundation provides comprehensive healthcare to all individuals. We provide services such as primary care, behavioral health care, onsite pharmacy and lab services, to meet the needs of our patients. Sandhills also provides healthcare services in the community such as patient education, flu shots, and health screenings such as blood pressure, diabetes, and cholesterol and HIV.

What is a Patient Centered Medical Home?

A “medical home” is a medical office or health care center where a team of health professionals work together to provide a new, expanded type of care to patients using medical technology to provide the best possible care for you in their office.

Your team may include a doctor, nurse, as well as other health professionals, like a pharmacist or behavioral health therapist. These team members will help you get healthy, stay healthy, and get the care and services that are right for you.

Your family caregivers are also part of the team, and YOU are the most important member!

Sandhills provides services such as primary care, behavioral health care, medical case management, onsite laboratory services, infectious disease treatment, disease management, pharmacy, Best Chance Network, physicals, and onsite fitness center. Sandhills Medical Foundation, Inc. is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employer under 42 U.S.C. 233(g)-(n). We are an FTCA deemed facility. Malpractice coverage is provided under the Federal Tort Claims Act (FTCA) pursuant to 42 U. S. C. 233(g)-(n).

Health Center Program Fundamentals

     · Located in or serve a high need community (designated Medically Underserved Area or Population). Find MUAs and MUPs

     · Governed by a community board composed of a majority (51% or more) of health center patients who represent the population served. More about health center governance

     · Provide comprehensive primary health care services as well as supportive services (education, translation and transportation, etc.) that promote access to health care.

     · Provide services available to all with fees adjusted based on ability to pay.

     · Meet other performance and accountability requirements regarding administrative, clinical, and financial operations.
