Bring a list of your current medicines or bring your medicines with you
Medicaid, Medicare, or private insurance
Check stubs, Social Security income statement, unemployment benefits statement, W2, or current tax return
Names, birth dates and social security numbers of your household members. Proof of guardianship if the appointment is for a child and you are not their custodial parent.
