Health Navigator Coordination

Sandhills Medical Foundation, Inc. has a health navigator to assist with Medicaid Unwinding and Affordable Care Act- Health Insurance Marketplace healthcare coverage. The health navigator assists the individual in an insurance program that right for you at any of our health centers.

The health navigator is also available to help individuals enroll in the Marketplace at small businesses, community events, and churches. Need to reapply? No problem, we are here to assist in any way.

Quick Facts About Medicaid Unwinding

1. Medicaid Unwinding means recertifications/reviews are going back to normal. Medicaid populations will be required to recertify every year.
2. Medicaid recipients could lose benefits as early as April 1, 2023.
3. Update your contact information – Make sure SC Medicaid or CHIP program has your current mailing address, phone number, email, or other contact information is current. This way, they’ll be able to contact you about your Medicaid or CHIP coverage.
4. Check your mail – South Carolina Medicaid or CHIP will mail you a letter about your Medicaid or CHIP coverage. This letter will also let you know if you need to complete a renewal form to see if you still qualify for Medicaid or CHIP.
5. Complete your renewal form (if you get one) – Fill out the form and return it to South Carolina Medicaid right away to help avoid a gap in your Medicaid or CHIP coverage.
6. Update your contact information by visiting https://tools.apply.scdhhs.gov/quick-tools/, calling (888) 549-0820 Monday through Friday from 8am to 6pm, or by visiting a SCDHHS local eligibility office (a list is available at gov/site-page/where-go-help.

If you no longer qualify for Medicaid or CHIP, you may be able to get health coverage through the Health Insurance Marketplace®.
